Mary's Meadow

Holyoke, Massachusetts

Project Overview

The Sisters of Providence wanted to provide skilled nursing home and rehabilitative care for their sisters and other residents of the Holyoke community. They began their exploration as part of the promise made to the sisters in 1997. In 2007 that promise was brought to fruition with the opening of Mary’s Meadow, the first small house in Massachusetts. Mary’s Meadow is comprised of four 10 Elder small houses, each connected to a central chapel, and each with a specially landscaped attached private garden.

Outcomes

For 3 consecutive years Mary’s Meadow at Providence Place has received the Excellence in Action award from My InnerView, the nation’s leading provider of performance-improvement solutions for the senior care industry. The Excellence in Action award is presented to skilled nursing facilities that maintain a significant response rate and score in the top 10 percent of qualifying facilities on the question “Would you recommend this facility to others?” with respondents rating the facility as “excellent.”

Mary’s Meadow also has earned a perfect, deficiency-free survey from the Massachusetts Department of Public Health (DPH) for three years in a row and a five-star rating with the Centers of Medicare/Medicaid (CMS). They have high occupancy and a waiting list.
